The deadline of July 15 is fast approaching for submission of health forms. The forms can be downloaded at www.wesleyan.edu/healthservices/geninfo/forms/healthforms09.pdf . Requirements include a physical exam and documentation of certain immunizations mandated by the State of Connecticut.
